What is a D4 medical exam?
Whilst the term ‘medical exam’ may imply several hours spent in a doctor’s office, this examination is very straightforward and often takes as little as 15 minutes to complete.
Your examination will include a check of your main health indicators, including a vision assessment and a blood pressure test, followed by a review of your medical notes and a discussion of your overall medical health. This confidential discussion will be had one-on-one with your doctor and will include questions about any:
- Cardiac conditions
- Diagnosed neurological disorders such as epilepsy
- Sleep disorders
- Substance misuse or dependence in the past 3 years
- Serious psychiatric illness or cognitive impairment that will likely affect your driving
- Any other serious medical condition, including ones that may spread to the brain
During the exam, the doctor will fill in your D4 form and provide details of any conditions that could impact your ability to drive safely. It is important that you are as honest as possible with the doctor when discussing your conditions as if you are managing them through medication you will likely still be able to apply for your Group 2 licence, as long as you can meet the DVLA’s requirements for controlling your condition.
Who needs this exam?
This medical isn’t just reserved for drivers who transport passengers – it is required for any role that regularly calls for you to be in control of a large commercial vehicle, including a bus, HGV, or ambulance.
You will need a D4 medical examination when applying for your initial Group 2 licence, as the completed D4 form will need to be submitted as part of your application. You will also need to attend this examination if you are:
- Applying to renew your Group 2 licence at 45.
- Over 45, in which case a D4 will be needed every 5 years.
- Over 65, when you will need a D4 annually to maintain your licence.
What should I bring with me?
